Focusing on the Whole You
As a participant in the Rhyme & Reason Lifestyle Solutions Program, your Lifestyle Medicine Specialists work collaboratively with you to help you reach your wellness goals through lifestyle change. We walk alongside you on your journey to better health.
What is Lifestyle Medicine?
Lifestyle Medicine is a medical specialty that uses therapeutic lifestyle interventions as a primary modality to treat health conditions such as heart diseases, type 2 diabetes, obesity, autoimmune disease, stress, and mental health concerns. Lifestyle Medicine-certified clinicians are trained to apply evidence-based, whole-person, lifestyle change to treat and, when used intensively, often reverse such conditions. Applying the six pillars of lifestyle medicine—a whole-food, plant-predominant eating pattern, physical activity, restorative sleep, stress management, avoidance of risky substances, and positive social connections—provides effective prevention for these conditions.
Offering Our Wellness Coordination Services to You
Rhyme & Reason provides the services of wellness coordination, where the needs of the consumer come first. Our goal is to implement and evaluate a comprehensive wellness program for our recipients enrolled in Health Care Coordination support services. We advocate for our recipients to coordinate medical needs beyond what can be provided through routine doctor/health care visits, emphasizing a balanced approach to care, including physical fitness, nutrition, physical health, and mental/emotional health.
Six Pillars of Lifestyle Medicine
Evidence supports the use of a whole food, plant-predominant diet to prevent, treat, and reverse chronic illness.
Regular, consistent physical activity is an important part of overall health and resiliency.
Managing negative stress can lessen anxiety, depression, and immune dysfunction and leads to improved wellbeing.
Improving sleep quality can improve attention span, mood, insulin resistance and can reduce hunger, sluggishness, and more.
Positive social connections have beneficial effects on physical, mental, and emotional health.
Avoid Risky Substances
The use of tobacco and excessive alcohol consumption have been shown to increase the risk of chronic diseases and death.
We see any patient who is wanting to make changes to their lifestyle (nutrition, exercise, stress reduction, weight loss, smoking cessation, anxiety, autoimmune issues, etc.). We do not provide primary care.
We do accept several insurance plans (see below). For any costs that are not covered by your insurance, you will receive a bill. Flexible Spending or Health Savings Accounts may be used to pay for these services as well.
We take Traditional Medicaid, Medicare, and Private Insurance.
Lifestyle Medicine complements traditional care; it does not replace it. We are happy to collaborate with your primary care provider for your care.
Lifestyle Medicine Assessments – Our appointment begins with an assessment of where you are for each of the pillars of Lifestyle Medicine. This assessment allows us to work collaboratively with you to choose goals and formulate a plan to work towards these goals.
Health Coaching – Our patients drive the change. We walk alongside you on this journey. We provide support and guidance throughout your journey toward better health.
Laboratory review and consultation – When necessary, we order lab tests individualized to each patient’s health concerns and provide a consultation on the results and best path forward.
What is Wellness Coordination?
Our Wellness Coordinator provides development, maintenance and routine monitoring of a participant’s individualized support plan and the medical services required to manage their health care needs.
Wellness Coordination services extend beyond services provided through routine doctor/health care visits required under the Medicaid State Plan and are specifically designed for participants requiring assistance of an RN/LPN to properly coordinate their medical needs.
